Program and Services
This foundation was founded for children with a critical cancer. Our programs and services start when a family gets approved by our board members. Initially the family is referred to us by the hospital social workers, nurses or doctors, then the family application is filled out and sent to us by the hospital social worker. It is important to know that we require the social worker to fill out the family application and assist the families in collecting the appropriate documents.
Below is a checklist of 5 requirements needed by the family in order to make our approval process work quickly and efficiently so the ill child's family can focus on their child's health instead of their past due bills and or financial issues.
1 - Child’s physician signature
2 - Social worker signature
3 - Copy of child’s birth certificate or social security card
4 - Parents contact information
5 - Parents authorization signature
Please note that we require the child's physician and social worker
